These forms can be filled out in advance and brought with you to your scheduled appointment. I will have a more streamline process in the future to reduce and hopefully eliminate the use of paper. The Minor Consent Form would accompany the corresponding intake form for your child. Please contact me is you have any questions.
Changes to your appointment must be made 24 hours in advance for massage clients and 48 hours or more in advance for Hypnotherapy and Coaching clients. You may cancel or reschedule online with 48 hours notice however you must contact me via phone, text or email so that I may cancel the appointment if you need to do so within 48 hours of your appointment. Any cancellations within the set time frames will be charged the full amount for their session. Any exceptions to this policy will be looked at on an individual, case by case basis.
Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show.” They will be charged for their “missed” appointment, (certificates that were to be used for appointment will be redeemed). New client no-show's forfeit any first session discount and any deposits paid.
If you arrive late, your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, I will then determine if there is enough time remaining to start a treatment. Regardless of the length of the session, payment in full is expected for the time you've reserved. For Hypnotherapy and Coaching clients the additional time, if any, will be due in full as a result of a late arrival.
Out of respect and consideration to your therapist and other customers, please plan accordingly and arrive on time.
